The Centers for Medicare & Medicaid Services (CMS) has published a Measure Information and Justification Form (MIJF) and Instructions, which combines the items from the Measure Information Form and Instructions (MIF) and the Measure Justification Information Form and Instructions (MJF) templates. The goal of combining the forms is to reduce duplication of information submission and burden on deliverables for CMS measure developers. CMS-contracted measure developers are the primary users of this form, which is a guide for documenting specifications and measure development information when submitting contracted measure deliverables to CMS. Please note: Submission of the MIJF is voluntary for non-CMS-contracted measure developers.
